Job Description:

Join our team as a Senior QOF Administrator and play a vital role in supporting patient safety and high-quality care. This is a fast-paced and rewarding position where you’ll coordinate recall processes, manage patient communications, and work closely with clinical teams to ensure patients receive timely follow-up care.

Main duties of the job

  • Work with our Digital Transformation Lead to maintain the practice recall system for all long-term condition reviews (e.g., diabetes, COPD, asthma, hypertension, mental health).
  • Generate and monitor QOF and practice performance reports.
  • Contact patients by phone, text, self-book links to arrange appointments, using self-book links etc.
  • Be competent with bulk messaging using Accurx.
  • Ensure that patients receive timely invitations and follow-up reminders.
  • Work closely with clinical staff to support accurate coding and data quality.
  • Identify patients overdue for reviews and coordinate proactive outreach.
  • Support annual QOF preparations and year-end submissions.
  • Assist with administrative tasks related to LTC management and population health.
  • Be competent using excel and word and running searches in EMIS.

We are seeking a dedicated and detail-oriented Long-Term Conditions Administrator to join our General Practice team.

The successful candidate will play a critical role in supporting the management and administration of services for patients with long-term health conditions. This position is ideal for a proactive individual with excellent communication and organisational skills who is committed to enhancing patient care.

Job Responsibilities:

Patient Coordination and Support:

Book, confirm, and follow up on appointments for patients with long-term conditions, ensuring regular reviews are conducted as per clinical guidelines.

Serve as a point of contact for patients, providing information on appointments, medication reviews, and health checks.

Assist patients with queries related to their long-term conditions, including explaining the review process and other relevant information.

Data Management:

Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with data protection regulations.

Update medical records with relevant information related to long-term conditions, health checks, and outcomes from clinical reviews.

Monitor and track key performance indicators, including the completion of reviews and outcomes, to ensure adherence to quality standards.

Collaboration with Clinical Team:

Work closely with HCAs, practice nurses, GPs, and other healthcare professionals to coordinate patient care and ensure smooth patient pathways.

Prepare and circulate patient lists for clinicians based on review schedules and clinical requirements.

Assist in the identification of patients who may require additional support or intervention based on their health records and clinical guidelines.

Administrative Support:

Ensure timely distribution and completion of recall letters, texts, and emails to patients requiring annual or routine health reviews.

Partake in the recall system for long-term conditions, ensuring that patients are scheduled for reviews in line with practice protocols.

Send results letters to patients following health checks.

Qualifications

  • LTC/QOF Recall Experience
  • Ability to run searches in EMIS
