Job Purpose
To manage and coordinate statutory Education, Health and Care (EHC) assessment and review processes for children and young people aged 0–25 with Special Educational Needs and Disabilities (SEND).
The postholder will ensure that Education, Health and Care Plans (EHCPs) are developed, maintained, and reviewed within statutory timescales in accordance with the SEND Code of Practice. This includes working closely with education settings, health and social care partners, parents, and other agencies to ensure that appropriate and effective provision is in place to meet individual needs.
Key Responsibilities
EHCP Assessment and Coordination
- Receive, analyse, and interpret professional reports and advice from education, health, and social care services.
- Make informed recommendations to Local Authority panels regarding appropriate provision and EHCP content.
- Prepare, draft, and finalise Education, Health and Care Plans within statutory deadlines.
- Coordinate annual reviews and ensure EHCPs are accurately updated following review outcomes.
Multi-Agency Working
- Act as a key point of contact between families, schools, and external agencies.
- Attend and contribute to multi-agency meetings, including EHCP panels, transition reviews, and LAC reviews.
- Ensure effective communication between all professionals involved in a child or young person’s support.
Case Management and Monitoring
- Manage an allocated caseload of children and young people with SEND (0–25).
- Monitor progress, timelines, and statutory compliance for all assigned cases.
- Maintain accurate records and case information systems.
Provision Planning and Placement Support
- Support place planning activity and maintenance of waiting lists for specialist provision.
- Contribute to decisions regarding school placements and specialist support provision.
- Work with panel recommendations to ensure suitable educational placements are identified.
Advice, Guidance, and Training
- Provide guidance to schools, SENCOs, parents, and professionals on SEND processes and provision.
- Contribute to and support delivery of training sessions for education settings and partners.
- Promote understanding of SEND legislation and local procedures.
Performance, Reporting, and Compliance
- Contribute to statutory returns, performance monitoring, and data reporting requirements.
- Assist in preparing information for inspections (e.g. Ofsted) and tribunal cases where required.
- Maintain accurate statistical and case management data.
