We are recruiting a full‑time Lead Reablement Worker in Ely, Cambridgeshire. The Reablement Service delivers short‑term, person‑centred home care to adults aged 18 and over, enabling independence while reducing reliance on long‑term care.
Responsibilities
- Assess individual needs and create tailored reablement plans that focus on recovery and independence.
- Work hands‑on with service users, helping them relearn essential daily living skills such as personal care, mobility and meal preparation.
- Monitor progress, review plans and adapt support to ensure goals are met.
- Collaborate with health and social care professionals, families and other agencies to provide a holistic approach to care.
- Maintain accurate records and documentation to meet compliance and quality standards.
- Manage a varied caseload to deliver timely, high‑quality support and positive outcomes.
Qualifications
- Proficient written and spoken English for case recording.
- NVQ Level 3 or equivalent apprenticeship (or equivalent experience).
- Experience in supporting individuals with care and support needs.
- Knowledge of the Care Act and CQC requirements.
- Strong communication and interpersonal skills, including experience liaising with health professionals and families.
- Ability to manage and prioritise a diverse caseload in a fast‑paced environment.
- Excellent verbal and written communication and IT skills.
- Awareness of equality, diversity and inclusion and commitment to confidentiality and safety.
- Access to a car, a full driving licence and business insurance; ability to travel across Cambridgeshire.
