Community Crisis Support Worker – 24/7 Home Treatment

Devon Partnership Trust is looking for a dedicated Community Support Worker to join the North Devon Crisis Resolution Home Treatment Team. This role requires 37.5 hours of work over 5 days each week, focusing on providing intensive community-based support for individuals experiencing mental health crises.

The successful candidate will work flexibly as part of a multi-disciplinary team and will play a key role in implementing care plans that help avoid hospital admissions, ensuring the welfare and safety of service users in the community.
