Job Summary
An exciting opportunity for a band 3 Community based Health Care Support Worker to work in Liverpool Diabetes Partnership, providing community based Diabetes clinics across Liverpool.
Responsibilities
- Assist the registered clinician in providing the highest standard of care for patients and their families/carers.
- Participate in all aspects of patient care.
- Provide efficient, effective and confidential administrative support to Community Diabetes Teams.
- Manage administrative tasks for clinics and education sessions, including scheduling and equipment set‑up.
- Handle incoming and outgoing mail in line with workplace procedures.
- Provide information to enable evaluation of care effectiveness and maintain accurate records.
- Offer reassurance and support to patients, families and carers.
- Work collaboratively with other professionals and agencies to meet patient needs.
- Recognise and respond appropriately to urgent and emergency situations.
- Perform general observations such as blood pressure, pulse, urinalysis and, where appropriate, undertake venepuncture under clinician direction.
Qualifications
- Good communication skills and motivation to work to a high standard in clinical and administrative services.
- Experience of working in a Health Care setting is desirable.
- Ability to travel across the city and a flexible approach to working hours.
- Demonstrated ability to follow health and safety procedures relevant to the area of work.
