Salary: Grade 5 salary scale: £25,614 – £31,325 per annum

Hours: 40 rota shift working hours (including breaks)

Contract Status: Permanent

About the service

Extern’s Multidisciplinary Homeless Support team (MDHST) has operated within the Belfast area since 2006. It provides a specialist service for individuals and families who are homeless or at risk of homelessness and who also experience additional complex needs including addictions, mental health difficulties, learning disability, and family breakdown. The team offers a range of professional interventions, has developed successful partnerships with core agencies across statutory, voluntary and community sectors, and advocates for appropriate service provision and support planning to realize the potential of each service user. The approach promotes change through a high tolerance, low threshold methodology, incorporating harm reduction principles.

This post will be based within our Floating Support service, which involves working with people with complex needs and those recently granted Refugee status in Northern Ireland. As a key worker you will offer a pragmatic and holistic assessment, care planning and structured ongoing support to people who are in their own accommodation and integrating into the community. The role involves working in partnership with service users in their home environment and includes practical assistance, evidence‑based interventions, advocacy, liaison with statutory and voluntary agencies, and assessing, managing and working effectively with risk.
