• Conduct assessments, care planning, and interventions for adults with complex needs.
• Promote independence and arrange tailored support.
• Ensure individuals receive the right care to enhance their quality of life.

Qualifications

• At least 3 years of post‑qualification experience.
• Social Work Qualification and registration with SWE (Social Work England).
• Experience with Court of Protection, safeguarding and Mosaic.
