Role Overview
One Coventry Intergrated Team is looking for two part‑time Band 2 Support Workers. They will provide reablement support for patients presenting with an acute deterioration in health and/or functional ability, as part of a community‑based, multidisciplinary team. The aim is to prevent avoidable hospital admissions and facilitate early hospital discharges by supporting patients through a reablement process.
Key Responsibilities
- Deliver reablement support to patients in the community.
- Identify when reablement is not achievable and signpost patients for suitable long‑term support.
- Collaborate with other professionals (Advanced Clinical Practitioners, Clinical Practitioners, Nurses, Care Co‑ordinator, Physiotherapists, Occupational Therapists, Social Workers, and other support workers).
- Maintain accurate records and report to occupational health or GP for vaccination and immunisation verification.
Qualifications & Requirements
- Valid UK driving licence.
- Access to a vehicle available for business use.
- Previous experience as a support worker or in a similar role is desirable.
- Availability for part‑time hours, up to 30 hours per week (5 days out of 7).
- Working a mixture of shifts: Early 08:00 – 14:00 / Late 16:00 – 22:00.
Training & Development
Training and induction will be provided, including a robust induction programme, one‑to‑one meetings, regular in‑service training, and clinical and managerial supervision.
