This permanent full-time role involves managing a caseload of care homes and residents, delivering high quality person-centred care. The post holder will work collaboratively with care home staff and the multidisciplinary team to improve healthcare access for vulnerable individuals.

The role requires an RGN with relevant community healthcare experience and offers a competitive salary between £45,953 and £54,254 per annum.
