Youth Transition Social Worker – Care Coordinator

Durham County Council is looking for a passionate Social Worker to support young people transitioning into adulthood. The successful candidate will lead the transition process, assess needs, and advocate for young people with Learning Disabilities, Mental Health needs, or Autism.
