Part-Time Health Care Support Worker

Job Summary

An exciting opportunity for two part‑time Band 2 Support Workers to join the One Coventry Integrated Team, a community‑based multi‑disciplinary team that delivers reablement support to patients presenting with an acute deterioration in health and/or functional ability.

Responsibilities

• Provide reablement support to patients, aiming to maximise independence and self‑management.
• Identify when further support is required and signpost patients to appropriate long‑term services.
• Collaborate with a wide range of health and social care professionals – including Advanced Clinical Practitioners, Clinical Practitioners, Nurses, Care Coordinators, Physiotherapists, Occupational Therapists, Social Workers, and other support workers.
• Participate in induction, appraisal and ongoing training, and receive clinical and managerial supervision.

Requirements

• Band 2 Support Worker qualifications and experience.
• Full, valid UK driving licence with access to a vehicle for business use.
• Current immunisation and vaccination report from a UK occupational health provider or GP.
• Previous applicants need not apply.
• Sponsorship cannot be supported for this vacancy.

Working Hours

Part‑time – up to 30 hours per week across 5 days out of 7. Shifts available: Early 08:00‑14:00 and Late 16:00‑22:00.
