The Coastal Primary Care Networkcomprising of the GP practices in Teignmouth & Dawlish have a vacancy for afull time Pharmacy Technician. We areseeking an enthusiastic individual to join our established team of Pharmacistsand Pharmacy Technicians and would welcome applications from those withexperience or newly qualified. You will have the full support of the ClinicalDirectors and will work closely with all our clinicians and our PCN Pharmacist& Primary Care teams.

There will be significant patientcontact, answering queries, conducting medication reviews and creatingpharmaceutical care plans; this will be Practice based and domiciliary working.This is an exciting opportunity due to the variety of the role and scope tomake it your own and for your personal and career development.

If you have experience within PrimaryCare that would be beneficial, but not essential. It is more important that you have the righttransferrable skills and experience, have exceptional attention to detail,excellent communication skills and are passionate about patient care andwellbeing, as well as inspired by the wider NHS plans and opportunities. It is a requirement of the role that you have either completed the CPPE Pathway for Pharmacy Technicians or ae willing to undertake this as part of the role.

Main duties of the job

To support the ClinicalPharmacist to ensure the delivery of safe, effective and efficient systems forrepeat prescribing, repeat dispensing, medicines optimisation, reducingmedicines waste and maximising patient outcomes.

The role may also includetraining and development of practice based prescribing coordinators andadministrators to support improved co-ordination and effective pharmacyadministration within general practice, especially with relation to repeatprescribing systems and processes.

Job responsibilities

To undertake medicines reconciliation inaccordance with practice protocols and to update patients medical records /prescribing accordingly.

To support the achievement of the practicesprescribing Quality and Outcomes Framework targets.

Working with the Clinical Pharmacist andPrescribing Co-ordinators to contribute to the review and development of repeatprescribing protocols / Standard Operating Procedures to improve the efficiencyand effectiveness of repeat prescribing systems.

To provide advice to GPs, staff and patients inchanges to prescribing to support the improvement of prescribing safety,quality and cost effectiveness.

To participate in medication audits.

To assist in the appropriate monitoring andmanagement of prescribing budgets and to liaise with the medicines managementteam at the Devon CCG.

To prepare evidence-based resources andinformation to support the implementation of rational cost-effectiveprescribing.

To support the Clinical Pharmacist to providemedication reviews, patient information leaflets, medicine awareness; and toundertake community visits as appropriate.

To take a proactive approach to the safehandling of prescriptions, including assessment of risk and makingrecommendations for improvement.

To respond to medication queries from patientsand staff in a professional manner referring to the appropriate GP inaccordance with practice protocols.

To advise on the sourcing and safe management ofmedicines as appropriate.

Patient services

To provide medication review services topatients via clinics in the practice, domiciliary visits and in residential andnursing homes, and to deliver pharmaceutical care plans that maximisecost-effective prescribing and improve the quality of patient care.

To present at patient group meetings or otherappropriate events to give advice on the appropriate use of medicines.

To produce patient information leaflets andposters and run medicine awareness projects throughout the year.

To prepare evidence-based resources andinformation to support the medicine management team and all other relevanthealth professionals in the implementation of rational cost-effectiveprescribing.

To help plan, develop and support theintroduction of new working processes within the practice to optimise thequality of prescribing.
