Overview

iPeople SC Solutions is currently recruiting for Support Workers for our client based in Wrexham and surrounding areas.

The successful post holder will work with service users with learning disabilities to enable them to fulfil their person-centred plans, promoting positive outcomes and maximise independence.

Duties & Responsibilities

  • Form, develop and maintain professional relationships with people who have disability, through direct supervision and shared activity, ensuring the person’s needs are met.
  • Assist and support individuals in day‑to‑day personal/practical care tasks according to individual needs (personal hygiene, dressing, cooking, household tasks etc.).
  • Assist and support individuals to access and accompany them in identified community based activities (e.g. leisure or day and employment services) in line with individual care plans.
  • Implement the service user’s individual care plan agreed by Social Services and Health Professionals.
  • Ensure standards of personal care and all aspects of safety are maintained at all times.
  • Assist Senior Support Workers to manage risk through implementation of risk assessments.
  • Participate in maintaining general household, personal and financial records in accordance with the Department’s policies and procedures under the direction and supervision of line management.
  • Complete accurate, concise and legible records that relate to the supported individual’s service delivery plan.
  • Communicate effectively and in the best interests of the supported person (reviews, team meetings, discussions with person & families, inter‑agency, staff and with manager via supervision).
  • Any other duties commensurate with the post.
  • Working hours: Shift rota, evenings, nights and weekends.

Requirements

  • Level 2 Health and Social Care Practice qualification (or equivalent relevant vocational qualification).
  • Moving and Handling.
  • First Aid Certificate.
  • Food Hygiene Certificate.
  • Good understanding of the needs of adults with disabilities.
  • Experience of supporting people who have a disability.
  • Enhanced DBS Certificate.
