Social Worker – Complex Psychosis Pathway (CPP)
An exciting opportunity has arisen for a highly motivated and enthusiastic social worker to work within the new Complex Psychosis Pathway (CPP) in Leeds. The CPP provides a whole‑system approach to Mental Health Rehabilitation and Assertive Community Treatment, including inpatient settings (Newsam Ward 5, Asket Croft, Asket House) and community teams.
Main duties
The post holder will oversee the social care needs of the service user group across the inpatient areas of the CPP and provide an innovative professional social work service to in‑patient service users while promoting social inclusion. The role involves:
- Participation in multidisciplinary and multi‑agency working, supporting colleagues and contributing to the development of complex rehabilitation.
- Work in partnership with service users, ward staff and partner agencies (housing, benefits, social services, etc.) to facilitate benefit claims and financial and housing support.
- Ensure the service user and families are central to the care plan and are fully involved.
- Attend review meetings, ward rounds, clinical meetings, and partner agency meetings to communicate and collaborate effectively.
- Support carers and champions, liaising with the local authority for Care Act assessments and supporting carers to access assessments.
- Complete statutory social circumstances reports for mental health tribunals and hospital manager hearings; produce other required reports.
- Maintain comprehensive, up‑to‑date written records of assessments, care plans, evaluations and ongoing process notes in line with Trust policy, and share appropriately with the MDT and external agencies.
- Coordinate with Social Work leads within the CPP and the Social Work pathway within the Trust.
- Support social work student placements.
Qualifications
- Professional registration as a social worker with Social Work England.
- Practice educator qualification, or working towards it.
Experience
- Experience of complex case management, multi‑agency working, and risk management.
Knowledge
- Knowledge of safeguarding vulnerable adults and managing this in a specialist mental health setting.
