Role Overview

We are seeking registered Social Workers to join our Hospital Discharge to Assess Team based in Central Manchester. The role is community‑based and involves conducting assessments for patients after hospital discharge, working within a multidisciplinary team to ensure a holistic approach to care. The position requires flexible availability, a fast‑paced working environment and the capacity to work 35 hours per week.

Working Hours & Contract Details

  • 35 hours per week
  • Permanent / Full‑time contract
  • Additional payments: 6.7% Flexibility Payment
    • Essential Car User Allowance – applicant must hold a full valid driving licence and provide own car for work.

Key Responsibilities

  • Conduct community assessments for patients discharged from hospital.
  • Work at the discretion of local, crisis response, intermediate care, and hospital teams.
  • Collaborate with a multidisciplinary team to deliver a holistic assessment.
  • Maintain flexibility to work across a 7‑day service.

Essential Requirements

  • Social Work qualification and registration with Social Work England.
  • At least 3 years reference history where possible.
  • Medical clearance via Pre‑placement health assessment (PPHA).
  • DBS check – enhanced DBS required, covered under the Rehabilitation of Offenders Act.
  • Provision of own car, full licence and eligibility for Essential Car User Allowance.

Desirable Qualifications & Experience

  • Hospital experience.
  • Practice educator experience.
  • BIA or willingness to work towards BIA qualifications.
