Job Title: Social Worker – Adult Hospital DischargeLocation: HaltonRate: £35 per hour
Liquid Personnel is currently recruiting for a Social Worker to join an Hospital Discharge team based in Halton.
Responsibilities
- Facilitate safe and timely hospital discharges, especially during the winter months.
- Manage a complex caseload independently and efficiently, ensuring each case receives appropriate attention.
- Support individuals and families transitioning from hospital to community care, helping them navigate this critical period.
- Collaborate with multidisciplinary teams to ensure seamless, person-centred care and that individuals’ needs are met.
- Demonstrate a proactive and compassionate approach, dedicated to improving the lives of those served.

Qualifications and Experience
- Social Work England registration
- Eligible to work in the UK
- Qualification in Social Work at degree level
- Post-qualification experience
