Hospice Social Worker – End-of-Life & Bereavement Care

Eden Valley Hospice is looking for a compassionate Social Worker to join our Patient and Family Support Team. This role involves delivering specialist palliative care support to patients and families, undertaking holistic assessments, and contributing to education initiatives. Ideal candidates will have a Social Work degree, experience in palliative care, and the ability to work collaboratively within a multidisciplinary team.
