Empowering Care Leavers Support Worker (Hybrid)

Stockport Council is seeking an Intensive Support Worker for the Leaving Care Team to provide tailored support to young people aged 16–25 who have been in care. You will help them navigate their transition to adulthood while collaborating with a multidisciplinary team.

This temporary role offers a hybrid working environment and requires skills in building trusting relationships with young people with complex needs. The position is critical in ensuring that care leavers have stable and supportive pathways into independent living.
