Early Intervention & Prevention Hub – Social Worker

Pay rate: £35 Umbrella

This is a fast‑paced team environment, so it’s important that applicants are confident hitting the ground running.

Responsibilities

• Managing a caseload of approximately 10-12
• Home care cases
• MCA work
• Safeguarding
• Hospital discharges
• Community assessments

Working pattern

Initially fully onsite. Once settled, option to work 2 days from home / 3 days onsite.

Requirements

• Relevant Social Work Qualification
• Registration with Social Care Wales
• Enhanced DBS registered on the update service or willingness to apply for DBS
• 3 years of referencing
• Driving licence with access to a car
