We are seeking a Discharge Support Worker to join our Mental Health Floating Support team, within wards in Greater Manchester. Multiple sites are available across North and South Manchester, including local community outreach.

This role provides a highly-responsive hospital in-reach service, delivering housing-related support for people with mental health needs and other barriers to discharge such as homelessness, a history of offences, or financial issues.

Our person-centred approach encourages service users to pursue their aspirations, supporting them in achieving better quality of life. You will help individuals move towards outcomes across key areas including independent living, social skills, community links, social inclusion, and pathways to meaningful employment.

The Role Includes The Following Responsibilities

  • Carrying out person-centred assessments to identify housing support needs
  • Ensuring clear targets for outputs, timescales and quality
  • Providing direct support and practical assistance in accessing housing and support options
  • Providing support to maximise financial wellness
  • Creating links with housing and support providers to build sustainable pathways
  • Ensuring smooth transitions out of hospital

Due to the nature of this role, experience in supporting individuals with mental health needs is an essential requirement.
