The Nelson Medical Practice is a dynamic, forward thinking and friendly practice and we have an exciting opportunity for a Pharmacy Technician to join our established Clinical Pharmacy Team. We operate from a large modern purpose built building based in Wimbledon Chase, we have an experienced and stable clinical workforce with a keen focus on high quality patient care. Our Clinical Pharmacy Team is committed to improving health and putting patients first. The key qualities we are looking for in a pharmacy technician are the ability to work as part of a team as well as independently. The successful candidate will work as part of a multidisciplinary team to support safe, efficient and effective medicines optimisation. You will play a vital role in improving patient safety, supporting prescribing quality within the practice.

Main duties of the job

The main duties and responsibilities of the role are:

  • Medicines optimisation, assisting with medication queries from patients,care homes and community pharmacies
  • carry out medicine reconciliation for discharge letters,outpatient letters and new patient registrations
  • Identify and resolve discrepancies and liaise closely with with Pharmacists and GP’s
  • Promote medicines synchronisation and encourage patient adherence
  • Support the implementation of local and national prescribing initiatives,including performing medication switches as directed by prescribers or pharmacists
  • Maintain MHRA alerts and high risk drug monitoring records
  • Support safe prescribing support recall of patients when needed.
  • Contribute to medicines safety alerts implementation
  • Lead and contribute to audits and quality improvement relating to the effective prescribing of medicines
  • Manage medication related queries and elevate as appropriate
  • Analyse prescribing data and share with the wider team
  • Support patient education and promote safe care and patient self management
  • Engage in continuous professional development and attend clinical and educational meetings within the practice
  • Adhere to practice policies,clinical governance frameworks,and safeguarding procedures.

Job responsibilities

The Pharmacy technician will work as part of the multidisciplinary general practice team to support safe,effective and efficient medicines optimisations. You will play a vital role in improving patient safety,supporting prescribing quality and ensuring robust medicines governance systems are in place within the practice.

  • Medicines optimisation – Support safe and efficient repeat prescribing processes and ensure accurate processing of prescription requests with queries from different sources
  • Assist with medication queries from patients,care homes,community pharmacies and prescribers and forward recommendations for alternatives.
  • Carry out medicines reconciliation for discharge letters,outpatient letters and new patient registrations.
  • Identify and resolve discrepancies and liaise closely with pharmacists and GP’s
  • Promote medicines synchronisation and encourage patient adherence
  • Support the implementation of local and national prescribing initiatives,including performing medication switches as directed by prescribers or pharmacists
  • Medicines Governance – maintain MHRA alerts and high risk drug monitoring records
  • Support safe prescribing and monitoring of high risk medicines and support the recall of patients where needed in accordance with local SOP’s
  • Education, training and supervision – Undertake the CPPE ( Primary Care Pharmacy Education Pathway), if not already completed.
  • Provide training and guidance to non clinical staff as needed, to support prescription processes within the practice
  • Quality Improvement – Lead or contribute to audits and quality improvement related safe and effective prescribing of medicines,using EMIS our clinical system.
  • Analyse prescribing data and share the learning with the wider team
  • Patient facing activities – Manage medication related queries and escalate appropriately
  • Provide advice and information to patients on safe medicines use
  • Support patient education, adherence and self care and promote patient self management
  • Professional and Organisational – Maintain GPhC registration
  • Adhere to Practice policies,clinical governance frameworks and safeguarding procedures
  • Develop and maintain effective relationships within the team and also with local stakeholders eg, ICB team, community pharmacists and hospital pharmacy teams
  • Engage in continuous professional development and attend clinical and educational meetngs within the practice.

Qualifications

  • You must have professional registration with with GPhC.
  • Qualification of a relevant nature i.e. FdsC in Medicines Management/Pharmacy services; BTEC level 4 diploma in Clinical Pharmacy Services and Therapeutics or a certificate of Higher Education in Pharmacy Technician Practice, as well as GCSE or equivalent including English and Maths ( grade C or above / Grade 4 or above).
  • Be committed to CPD and completion of the CPPE pathway ( if not already completed)
  • Experience of working as a qualified Pharmacy Technician in primary Care, PCN,Community Pharmacy or hospital setting.
  • A good understanding of Primary Care services and the NHS structure
  • Knowledge of GDPR, confidentiality and safeguarding
  • An organised approach to work
  • Excellent communication skills both written and verbal
  • Able to work as part of a team and independantly
  • Experience of working in a Primary Care setting
  • Experience of using EMIS – Clinical system
  • Experience supporting audits and quality improvement initiatives
  • Experience dealing with patient queries and providing advice within scope of practice
  • Knowledge of local prescribing guidelines
