South Bristol locality has been chosen as one of the 43 places to be included in the first phase of the National Neighbourhood Health Implementation Programme (NNHIP), a new and exciting national project. Within South Bristol, there are three pilot projects running, aligned with the local Primary Care Network (PCN). Partners in the Swift PCN geography originally agreed to focus their project on building on the successful delivery of mental health group sessions provided by Mental Health Workers based in the GP practice, jointly funded by Avon and Wiltshire Mental Health Partnership (AWP). Due to the recent allocation of funding, the original proposal has developed into four bespoke sessions focusing on the contribution of positive mental health to wellbeing, and four further sessions hosting partners to help address the wider determinants of health such as housing and food poverty. The target cohort across South Bristol is aligned with the recommendations of Healthier Together 2040 report, with the cohort being adults aged 20‑64 diagnosed with at least three of the following: diabetes, hypertension, pain, anxiety and depression.
For the project’s success, a project manager is required to oversee and lead on the implementation of the project. The project manager will work closely with the care co‑ordinator to lead and deliver on the project‑associated work. Whilst the project manager will be employed by the PCN, they will be working closely with local partners.
