Community Mental Health Nurse – MHSOP

We are looking for a community mental health nurse to provide holistic assessment and treatment pathways for older adults with mental health and frailty needs. The role focuses on seamless pathways between primary and secondary care for functional mental illness and dementia care.

As part of a multi‑disciplinary team, the nurse will work alongside Clinical Psychologists, Consultant Psychiatrists, Occupational Therapists, Social Workers and other Mental Health Nurses, and will participate in CPD, reflective practice, supervisions and academic training.

The post holder is required to take responsibility for the assessment of care needs, development, implementation and evaluation of programmes of care, setting standards of care and managing a defined caseload. The role includes liaising with other agencies, supervising and training staff and students, and acting as the care coordinator to ensure care plans are met. The work is carried out within a multi‑disciplinary team and represents Hertfordshire Partnership NHS Foundation Trust in relationships with service users, carers, relatives, charities and providers.

Person Specification

Knowledge, Training and Experience

  • Degree, diploma or equivalent in Nursing (Mental Health Nursing).
  • Registration with the Nursing and Midwifery Council.
  • Few years of post‑registration experience in Adult Mental Health Care.
  • Pre or post‑registration training/short courses in mental health.
  • Experience in community and inpatient nursing.

Areas of Experience and Knowledge

  • Knowledge of legislation related to mental health and social care (e.g., Mental Health Act, Mental Capacity Act, Care Act, Care Programme Approach).
  • Good knowledge of Microsoft Word, PowerPoint and Excel.
  • Ability to manage distressing and challenging situations.

Salary

£41,957 to £50,387 per annum pro rata (inclusive of 5% HCAS).

Contract

Permanent.
