Community Integrated Care is recruiting for an Advanced Support Worker with flexible shift patterns. This is an exciting opportunity for a skilled and experienced support worker to step up. You will support adults with learning disabilities and autism, who have complex needs, to lead the best life possible.
Responsibilities
- Provide person-centred support to adults with learning disabilities and autism, often with complex needs, staying proactive and capable of decision-making and liaison with family and professionals when required.
- Support people to work towards person-centred outcomes, helping them develop independence and maintain safety in their home and community.
- Spend your interaction time with the person you support, making the biggest impact on their lives.
Requirements / Qualifications
- Experience in supporting people with Autism, Learning Disabilities, ADHD, Epilepsy and medication management and administration (where applicable).
- Experience with complex care needs, youth transition and supporting young adults who are care leavers.
- Ability to work in a supported living environment with flexible shifts; full UK driving licence may be required depending on role and location.
About The Service
- Brackley House is a five-bedroom property in the Heaton Chapel area of Stockport, supporting three young men aged 19–24 with complex care needs who live there.
- Brackley House is a supported living project for four young men; some are care leavers who have transitioned into their own tenancies.
- People supported require help with daily activities, including personal care, housekeeping, meals, cooking, and community participation.
