There is an exciting opportunity to join the community Nursing Teams based in Stanton Suffolk covering a rural site as a Band 6 Junior Sister. we have 1.2WTE available we would welcome applicants for full and part time hours. This is an ideal post for those looking to be supported to develop from a Band 5 to a Band 6 leadership role as Junior Sister.
The nursing teams form part of the Integrated Neighbourhood Team (INT) and would see the successful applicant joining one of our dynamic and friendly team of community nurses, therapists, community matrons, EIT and Virtual ward, and colleagues across social care.
You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multidisciplinary team including the Community Matrons, tissue viability team and diabetic nurse specialists.
Within your role you will be able to provide clinical and supervisory leadership to staff and students within the team. Supporting the management of the nursing staff by taking responsibility, in partnership with the District Nurse, for the day-to-day co‑ordinating, planning and delivery of the nursing services within the INT.
Main duties of the job
You will be supporting the management of patients with multiple and complex needs, using evidence-based practice to assess, plan, implement and evaluate interventions to improve patient health.
You will be using a wide range of nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting.
Experience in leadership as well as clinical experience in palliative care, wound management and catheter management will be an advantage. INT quality improvement project work and conducting monthly clinical audit will also be a key part of this role.
Education & Qualifications
- Current NMC registration or working towards
- Leadership training
- Level 3-degree Clinical module on long term conditions such as Leg Ulcer Management, Diabetes care or End of Life Care.
- Care Lead / Case Management/Health coaching training.
- Evidence of continuing professional development recent and relevant to the post.
- Demonstrate knowledge, understanding and use of HR management policies including Capability, Sickness management, and Disciplinary.
Experience & Knowledge
- Evidence of working within the community nursing setting.
- Experience of supervision of staff, including PDR
- Evidence of high level of ability to reflect on and critically appraise own practice and performance.
- Experience of co-ordinating and delegating work and reviewing others performance.
- Experience and knowledge of autonomous and independent decision making in home environments, which are often complex and unpredictable.
- Enhanced awareness of mental health conditions and issues.
- Comprehensive holistic assessment skills in the patient’s home environment
- Evidence of formal clinical supervision.
- Ability to give constructive feedback and praise

- High level of nursing skills and knowledge for managing and supporting the care of people in their own home environment.
- High level of ability to work under pressure and set priorities to achieve service objectives.

- The ability to drive to rural locations and hold a current valid driving licence.
