An exciting opportunity has arisen for a confident proactive individual within Tower Hamlets Community Health Teams. A rehabilitation support worker is required to assist physiotherapists/occupational therapists in providing treatment to patients. The post holder will be responsible for carrying out rehabilitation programs with patients, as devised by the physiotherapist/occupational therapist, to maximise patient’s mobility and independence in daily activities in their own home.
Main duties of the job
- Responsible for implementing and supporting rehab plans written by Physiotherapists and Occupational Therapists.
- To involve clients and carers in the planning of care/rehabilitation programmes and encourage self-management where appropriate.
- Contributes to client goal planning sessions and shows initiative in areas where competent and as agreed by senior staff.

Job responsibilities
- Under direction of the physiotherapist/occupational therapist, contribute to the provision of rehabilitation programs to clients referred with primarily physical disabilities.
- To be responsible for own time management and provision of rehabilitation programs with designated clients.
- To continually report client progress back to the physiotherapist/occupational therapist, both verbally and written.
- To be an active member of the multi-disciplinary team, liaising with nursing and therapy staff with regard to designated clients.
- To develop a range of rehabilitation skills working with clients with physical disability.

Education and Training
- NVQ Level 2 in Health and Social Care
- Care Certificate
- Previous community and patient experience
Knowledge and Skills
- Ability to undertake Manual Handling activities
- Have knowledge and experience of Rehabilitation/Therapy skills and activities

- Ability to work as part of a team and as a lone worker
