Overview

Senior AACHC Nurse Assessor – Birmingham NHS (Full-Time, On-Site)

Agency Contract – 2 Months

Day Rate: £320

Service Care Solutions is currently recruiting for a Senior AACHC Nurse Assessor to join an NHS team based in Birmingham. This is a full-time, on-site role with no remote working, offered on a 2-month agency contract.

Responsibilities

  • Manage a complex clinical caseload including assessments, 3/12 month FNC/CHC reviews, unscheduled reviews, Fast Tracks, Personal Health Budgets, and ongoing case management.
  • Conduct NHS-funded Nursing Care (FNC) determinations and ensure timely reviews in line with Department of Health guidelines.
  • Lead multi-disciplinary teams (MDTs) through CHC assessments and decision-making processes.
  • Undertake Continuing Healthcare reviews, fast track assessments, local review panels, and appeal assessments.
  • Case manage patients with complex needs including mental health, physical disabilities, learning disabilities, and those on transition pathways.
  • Ensure assessments are robust, transparent, and multi-agency where required.

Requirements

  • Previous experience in Continuing Healthcare assessments.
  • Strong background in working with patients with complex, long-term, or palliative care needs.
  • Proven ability to liaise effectively with multi-disciplinary teams.
  • Experience in audit and implementing outcomes.
  • Ability to work autonomously and apply evidence-based practice.
  • Commitment to partnership working across health, education, and local authority settings.
