Overview

The Advanced Clinical Practitioner (ACP) will provide advanced and expert level of practice (including non-medical prescribing) within the adult Community Mental Health Team, leading to improved patient outcomes. The ACP will act as a professional resource for the multidisciplinary team (MDT), patients and carers within the scope of their practice and competence. The ACP will understand professional accountability and legal frameworks for advanced practice and be professionally accountable and legally responsible for all aspects of own work. The ACP will demonstrate safe practice in line with Trust policies and procedures.

Responsibilities

  • The post holder will support the clinical and operational team to deliver service improvements in the quality of care and safety through service development, evaluation, and audit.
  • They will be an integral part of the MDT supporting clinical decision making and supporting service pathways, developing nurse-led clinics and providing brief interventions.
  • The post holder will also support with providing a seamless pathway into & out of the community mental health team (CMHT) once interventions/goals have been achieved, to promote recovery and self-management, as clinically appropriate.
  • The ACP will hold a caseload and will lead assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures.
  • The post holder may additionally lead on practice and service developments and is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.
  • To support improving workforce knowledge and practice through the contribution to a CMHT education and training programme across the community teams within Bolton.
  • To participate in the Bronze on site / Bronze on Call rota.
  • The core hours are Monday to Friday 9-5, although some flexibility, including weekend / BH working may be required.

Qualifications and requirements

  • Demonstrated advanced clinical practice with prescribing as part of the role within a CMHT setting, and ability to act as a professional resource for the MDT and patients.
  • Ability to understand and apply professional accountability and legal frameworks for advanced practice; be professionally accountable and legally responsible for own work.
  • Commitment to safe practice in line with Trust policies and procedures.
  • Experience in service development, evaluation and audit to support improvements in care quality and safety.
