  • Utilise a wide range of services, including third-sector organisations, to support people’s goals.
  • Ensure appropriate action is taken when individuals are at risk of harm, abuse, or neglect.
  • Provide clear information about the Charging policy to help individuals and families understand financial implications of Adult Social Care services.
  • Ensure oversight of people in short term community beds to expedite flow for our hospital discharges.

About You

  • Experience working in health and social care environments, ideally within a hospital or reablement setting or the third sector.
  • Proven ability to work in a fast-paced environment managing administrative systems and client interactions.
  • Strong communication and organisational skills.
  • Understanding of issues affecting people aged 16 and over, including older adults.
  • Forward-thinking and committed to delivering high-quality, person-centred services.
