We are currently recruiting for an experienced Band 5 Community Nurse (RGN) to join a busy and supportive Community Nursing Team across the Staffordshire area for a 12-week placement, starting ASAP.

The Role
As a Community Staff Nurse, you will play a key role in delivering evidence-based nursing care across a defined patient population. This dynamic role involves working across a range of community environments and collaborating closely with multidisciplinary teams to support patients with varying healthcare needs.
Your responsibilities will include:
Delivering care to patients in their own homes, residential homes, and clinic settings
Assessing patient needs and developing, implementing, and evaluating care plans
Managing a defined caseload within the community
Supporting hospital discharge and preventing unnecessary admissions
Working collaboratively with multidisciplinary teams and external agencies
Contributing to service development where required
Undertaking supervisory duties such as work allocation when appropriate
Requirements
Registered General Nurse (RGN)
Band 5 experience
Community and/or District Nursing experience
Strong assessment and clinical decision-making skills
Ability to work independently in community settings
Excellent communication and organisational skills
