We arelooking for an enthusiastic and motivated Clinical Practice Pharmacist to joinour established clinical pharmacy team at Colton Mill & The Grange MedicalCentre.

This isan exciting opportunity to work within a friendly, supportive andforward-thinking primary care team, helping to deliver safe and effectivemedicines optimisation for our patients.

Thesuccessful candidate will play an important role in clinical medicationreviews, prescribing safety, medicines optimisation and long-term conditionmanagement, working closely with GPs and the wider multidisciplinary team.

Thisrole offers an excellent opportunity for a pharmacist looking to developclinical skills in primary care within a well-supported environment.

Main duties of the job

The postholder will support the delivery of medicines optimisation services within thepractice. Responsibilities will include:

  • Undertaking patient-facing clinical medicationreviews to optimise medicines and reduce medicines-related harm
  • Supporting the management of long-termconditions through medicines optimisation
  • Conducting structured medication reviews andsingle-condition medication reviews where appropriate
  • Completing medicines reconciliation followinghospital discharge or transfer of care
  • Supporting repeat prescribing systems andprescription authorisation processes
  • Leading and supporting high-risk medicinesmonitoring systems
  • Responding to medicines-related queries frompatients and clinicians
  • Ordering and interpreting blood tests formedicines monitoring
  • Supporting prescribing safety, medicinesgovernance and audit activity
  • Providing patient counselling and medicinesadvice through clinics and telephone consultations

We carefor a registered population of approximately 14,000 patients and are part ofthe Cross Gates Primary Care Network, supporting a wider population of around30,000 patients.

Clinical Care and Medication Reviews

Undertake patient-facing clinical medication reviews to optimise medicines and improve outcomes.

Support the management of long-term conditions such as hypertension, diabetes, asthma, COPD and cardiovascular disease.

Identify patients at risk of medication-related harm, inappropriate prescribing or polypharmacy and implement improvements.

Provide patient counselling and support to improve medication adherence.

Prescribing and Medicines Management

Contribute to structured medication reviews and medicines optimisation initiatives.

Support safe prescribing within scope of competence, including independent prescribing where qualified.

Review repeat prescriptions and ensure appropriate monitoring and review processes are in place.

Ensure prescribing aligns with NICE guidance, local formularies and medicines optimisation policies.

Support deprescribing where medicines are no longer clinically appropriate.

Medicines Safety and Quality Improvement

Lead monitoring processes for high-risk medicines and ensure appropriate patient monitoring.

Implement prescribing changes following national patient safety alerts, MHRA updates and medicines supply issues.

Undertake clinical audits and contribute to quality improvement initiatives.

Identify patient cohorts at risk through prescribing data and clinical system searches.

Transfer of Care

Undertake medicines reconciliation following hospital discharge or transfer of care.

Identify and resolve discrepancies between hospital and primary care medication records.

Liaise with patients, carers and community pharmacies to ensure safe continuity of medicines.

Clinical Support

Provide clinical advice for medicines-related queries from patients and practice staff.

Review side effects, drug interactions and adherence concerns.

Order and interpret blood tests related to medicines monitoring where appropriate.

Multidisciplinary Working

Work collaboratively with the practice team and wider PCN pharmacy team.

Participate in clinical meetings and contribute to shared decision making.

Liaise with secondary care, community pharmacies, care homes and other healthcare services to ensure continuity of care.

Patient Support and Public Health

Provide medicines advice clinics and telephone consultations.

Promote safe and effective medicines use and support patients in self-management of long-term conditions.

Support public health initiatives including antimicrobial stewardship.

Person Specification

Qualifications

  • Masters Degree in Pharmacy (MPharm) or equivalent
  • Current registration with the General Pharmaceutical Council (GPhC)
  • Experience working as a foundation-level pharmacist
  • Independent Prescribing qualification or working towards
  • Good knowledge of medicines optimisation and safe prescribing principles
  • Excellent communication skills and ability to work effectively within a multidisciplinary team
  • Ability to work independently within scope of competence
  • Experience working in general practice or primary care
  • Experience undertaking clinical medication reviews
  • Completion of the CPPE General Practice Pharmacist Training Pathway

Experience

  • Foundation-level pharmacist experience with a professional practice portfolio.
  • Awareness of common acute and long-term conditions in primary care.
  • Experience conducting clinical or structured medication reviews.
  • Collaborative experience with general practice, community, and hospital pharmacy teams.
  • Proven ability to build and maintain professional relationships across healthcare organisations.
  • Demonstrable experience as a foundation level pharmacist evidenced through a professional practice portfolio
  • Demonstrates ability to work collaboratively with general practice, community pharmacy and hospital pharmacy teams
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations
