Part Associate Dentist required for 1 day per week in a busy, independently owned practice near Burscough, Lancashire.

Practice details

• Practice is an IV Sedation referral clinic
• £14 per UDA
• 1500 UDAs available
• 50% split on any Private & Labs
• 4 fully computerised surgeries, SOE
• Digital X-rays
• Rotary Endo
• Fully qualified nursing team & long standing Practice Manager, full support
• 3 Associates, Hygienist, Nurses (16= years service) and Manager on-site
• Surrounded by countryside and located close to the 2 train stations
• FREE STAFF PARKING ON-SITE
• Available April, start date negotiable

Compliance requirements

• Full NHS Performer Number
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
• IRMER
• PIN number
