This is an exciting opportunity to join a passionate and motivated, multi-disciplinary team of professionals who support people with dementia and their carers to remain living independently in the community. The team has a philosophy of early intervention and specialises in working with those who are hard to engage.
We become involved when situations are breaking down. The team aims to prevent unnecessary admissions to hospital and premature moves into long term care. There is an open referral system, and we work with people for time limited periods, aiming to stabilise situations and ensure there is support in place to meet their needs in the future.
The team has been focussed on quality improvement for both service users, carers and the staff team, so this is an opportunity to be a part of an improvement initiative. As a senior practitioner, you will help to shape the future of the service and assist in its leadership.
Main duties of the job
Working with other multi-disciplinary professionals within the team you will:
- Manage and hold a caseload
- Undertake initial assessments including risk assessments
- Engage and work with clients with dementia and their carers
- Liaise with statutory and voluntary sector to organise care and support as required
- Triage referrals and undertake duty role
- Attend Clinical and Business Meetings for the team
- Take on a supervisory role within the team
- Represent team in meetings with partner agencies, including INT meetings

Person Specification
Qualifications
- Registered Mental Health or Learning Disability Nurse, Occupational Therapist OR Social Worker with the relevant professional registration
Knowledge
- The ability to use clinical assessment tools and outcome measures effectively and collaboratively
- Up to date knowledge of legal and ethical frameworks including Mental Health Act
Experience
- Experience in working in team delivering person centred treatment packages
- Demonstrable experience in assessment of mental health
- Experience of supporting transitions
