We are a friendly and supportive training practice located on the outskirts of Halifax, West Yorkshire and we have a very exciting opportunity for a motivated, committed and experienced Deputy Care Navigation Lead to join the team for 37 hours per week.
Main duties of the job
Support CareNavigation Lead in day-to-day supervision of team.
Assist with staffrecruitment, induction & training
Help manage rotas,annual leave and sickness cover
Monitor teamperformance
Assist the CareNavigation Lead with appraisals, updating policies and protocols
Ensure Care Navigationteam duties are carried out efficiently
Assist the CareNavigation Lead with handling IT Issues
Oversee and completethe booking of appointments, handling of enquiries, and processing ofprescription requests by the team.
Identify and resolveoperational issues
Promote a welcoming,caring, and efficient front-of-house environment.
Support communicationbetween patients, clinicians, and administrative staff.
Identify and deliverteam training where required alongside the Care Navigation Lead.
Ensure annual leaverequests are appropriately made and recorded and sickness is recorded
Coordinate theprovision of temporary reception staff if required ensuring sufficient cover isprovided for periods of leave and other staff absences
Provide initialguidance and act as an initial port of call in dealing with patients who wishto complain
Manage all deliveriesto the practice, ensuring adherence to the cold chain policy
Ensure the stock ofrelevant paperwork is replenished
Deputise for the CareNavigation Lead during periods of absence
Assist with PremisesManagement at the practice sites
About us
The Practice
We are a two site practice based in a beautiful part of West Yorkshire and the successful candidate will be required to work across both of our sites. The surgery is well-established, friendly, supportive and inclusive. We have a growing practice size of approximately 16,500 patients currently based over the two sites which are located within ten minutes of each other.
We are committed to the well-being of our patients and staff and always welcome new ideas to improve patient care. We are very proud to have an excellent reputation in the local area, providing very high standards of care and currently rated Good by CQC. We take an active part in our PCN (Lower Valley PCN) and the LMC with one of our GP Partners currently the Clinical Director of our PCN and another GP Partners who is on the LMC board. The practice uses TPP SystmOne clinical system.
The Team
The team includes six GP Partners, two Salaried Gps, four Advanced Clinical Practitioners who are supported by a full Nursing Team comprising of four Practice Nurses, one Nursing Associate, one Healthcare Assistant, two Phlebotomists as well as Pharmacy Prescribers. The practice also has First Contact Physiotherapy and Podiatry services running from the practice.
