Hospital Team –• To provide an outcome focussed assessment service to all adults with physical disabilities or age related problems. • Local Authority and hospital discharge experience required. • Hybrid set up, min 2-3 days a week in the office • Must be a driver with a valid driving license and a vehicle to use for work
