Role Overview

We are looking for an experienced Care Manager to oversee the coordination and delivery of complex care packages, ensuring all clients receive safe, effective, and person-centred care. This role involves managing staff allocation, maintaining compliance, and ensuring high standards of service delivery.

Key Responsibilities

  • Manage and coordinate day-to-day operations of complex care packages
  • Ensure all shifts are safely staffed with suitable and competent personnel
  • Work closely with recruitment to ensure effective staffing pipelines
  • Maintain strong relationships with:
  • Clients and families
  • Case managers
  • Healthcare professionals
  • Monitor and ensure compliance with:
  • CQC standards
  • Company policies
  • Safeguarding procedures
  • Conduct regular reviews of care packages and staff performance

Essential Requirements

  • Experience in a care coordination or care management role
  • Strong understanding of complex care packages

Knowledge of:

  • Manual handling
  • Safeguarding
  • Risk assessments
  • Excellent organisational and time management skills
  • Ability to work in a fast-paced environment
  • Strong communication and problem-solving skills

Desirable

  • NVQ Level 3 or above in Health & Social Care
  • Experience working within a complex care or domiciliary care agency
  • Understanding of rota systems and workforce planning
