An exciting opportunity has arisen to join the Intermediate Care Service in Walsall. This is a multi‑disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care.
We are particularly looking for enthusiastic and committed individuals to join our team to shape and deliver our services and contribute to the model of best practice. Successful candidates will have experience working in Adult Social Care and will join our multi‑disciplinary team.
You will support customers in their own homes to develop or regain skills and confidence, co‑coordinate and deliver a programme of active Reablement to help customers achieve their full potential, working in accordance with individualised Reablement Plans. You will work cohesively within a multidisciplinary team to maximise customer independence and reduce the need for ongoing support, and support customers to enjoy full access to community resources and develop informal support networks.
About the role
- Support/enable activities of daily living (ADL’s) including personal care, as part of a responsive service in partnership with other agencies, ensuring that religious, cultural and gender needs are met, and personal dignity and privacy are maintained. All care interventions will be agreed with the service user and written within a support plan.
- Ensure that ADL’s are undertaken within the agreed parameters set out in the person’s support plan.
- Minimum qualification: NVQ Level 2 Health and Social Care or equivalent.
