NHS Health Check Clinical Development Nurse

The NHS Health Check Clinical Development Nurse will provideclinical leadership and oversight for the delivery of the NHS Health CheckProgramme across Leeds. The role will support the high-quality and consistentdelivery of the programme, ensuring that it aligns with national guidance andlocal population health priorities.

The requirement for this role is 7.5 hours per week.

Main duties of the job

The post holder will act as a clinical advocate for the NHS Health CheckProgramme, supporting GP practices to improve the uptake, quality, and outcomesof NHS Health Checks. The role will work in close partnership with the DeputyService Delivery and Relationship Manager, the OperationalService Delivery & Performance Lead, PublicHealth, Leeds City Council, and General Practice teams, includingadministrative staff, Healthcare Assistants (HCAs), Nurses and wider clinicalteams.

Job responsibilities

MAIN DUTIES

Provide clinical leadership and specialist expertise to support thedelivery, development, and continuous improvement of the NHS Health Checkprogramme across Leeds.

Promote best practice in cardiovascular disease prevention, earlydetection, and risk management in line with national guidance, NICErecommendations, and Public Health priorities from Leeds City Council.

Support clinicians and healthcare professionals in deliveringhigh-quality NHS Health Checks through the implementation and ongoingdevelopment of the quality assurance framework.

Act as a clinical champion for preventative health, advocating for thevalue and impact of NHS Health Checks and supporting their integration withinwider primary care and Public Health, Leeds City Council initiatives.

Support the development, implementation, and evaluation of NHS HealthCheck initiatives, pathways, and service improvements to ensure the programmeremains responsive to population health needs.

Promote and support quality improvement initiatives aimed atincreasing NHS Health Check uptake, improving clinical outcomes, and reducingnegative variation through the quality assurance framework.

Review performance and population health data to identify trends,inequalities, and variation in delivery, and work collaboratively with theoperational team and delivery partners to implement actions that supportservice improvement.

Provide clinical insight and practical support to the operational teamand general practice teams to help GP practices and Primary Care Networks (PCNs)meet programme Key Performance Indicators.

Work with GP practices and PCNs to review and strengthen StandardOperating Procedures (SOPs) relating to NHS Health Check delivery, supportingthe development of consistent and effective processes.

Work creatively with GP practices to identify realistic and practicalsolutions to barriers affecting NHS Health Check delivery, tailoring supportbased on individual practice needs, workforce capacity, and local populationhealth data.

Ensure the NHS Health Check programme is delivered in line withnational guidance, clinical standards, and local policies.

Support the development, implementation, and review of clinicalpathways, protocols, and referral processes associated with NHS Health Checks.

Contribute to internal clinical governance processes, including riskmanagement and quality assurance audits that support and strengthen theorganisations governance infrastructure.

Provide clinical advice, guidance, and support to staff delivering NHSHealth Checks through Leeds GP Confederations Enhanced Access Service andacross wider general practice delivery teams.

Deliver NHS Health Check training and contribute to the development ofhealthcare professionals working within general practice and wider partnerorganisations, including third sector organisations, community services, andsocial prescribing teams.

Promote awareness and understanding of the NHS HealthCheck programme across primary care and support services, ensuring a consistentlevel of knowledge and high-quality delivery of theprogramme.

Support integration between NHS Health Checks and wider preventionprogrammes such as smoking cessation services, weight management programmes,and the National Diabetes Prevention Programme.

Champion a population health and health inequalities approach bysupporting targeted outreach and engagement strategies that improve access toNHS Health Checks for underserved or higher-risk communities.

Engage with a wide range of stakeholders across primary care, PublicHealth, Leeds City Council, and community organisations, using strongcommunication and influencing skills to promote the value of NHS Health Checksand inspire partners to maintain prevention as a key priority.

Apply innovative, creative and flexible thinking to problem solving,working with stakeholders to develop sustainable and impactful approaches toprevention and cardiovascular health improvement.

Person Specification

Experience

  • Experience delivering the NHS Health Check programme.
  • Experience presenting information or facilitating training, workshops, or stakeholder engagement sessions.
  • Experience working within primary care, community health, or public health settings.
  • Experience supporting or delivering preventative health initiatives, including cardiovascular risk reduction, screening programmes, or population health interventions.
  • Experience using clinical judgement and evidence-based practice to support patient care and service delivery.
  • Experience working collaboratively with multidisciplinary teams, General Practice teams, PCNs, and external partners.
  • Experience interpreting and using performance or population health data to inform service development and improvement.
  • Experience contributing to clinical governance activities, including audit, risk management, and quality improvement.
  • Experience providing clinical leadership, training, or mentoring to healthcare professionals
  • Experience working with public health teams, local authorities, or third sector partners.
  • Demonstrable experience of working to reduce health inequalities and supporting underserved groups to access services and achieve positive health-based outcomes.

Qualifications

  • Registered Nurse with current NMC registration.
  • Degree in Nursing, Public Health, or a related health discipline, or equivalent experience.
  • Evidence of continuing professional development (CPD) relevant to cardiovascular disease prevention, public health, or primary care.
  • Training, leadership or coaching qualification, or similar experience. Qualification or training in Infection Prevention and Control.

Systems and Compliance

  • Experience working with clinical systems used in general practice (e.g. EMIS, SystmOne)
  • Knowledge of population health management approaches and data-driven service improvement.
  • Ability to work within information governance and data protection regulations, including GDPR and NHS confidentiality standards.
  • Experience supporting quality assurance frameworks or programme monitoring.
  • Understanding of NHS Health Checks Best Practice and NICE guidelines relating to cardiovascular disease prevention and risk management.
