PLEASE NOTE THAT WE WILL ONLY ACCEPT APPLICATIONS FROM STAFF CURRENTLY EMPLOYED BY BETSI CADWALADR UNIVERSITY HEALTH BOARDAn exciting opportunity has arisen for an enthusiastic and motivated Band 6 Deputy Home First Co-Ordinator to join the multi-disciplinary Home First Team based in Wrexham. Now is a great time to join the Home First Team, as we further expand and develop the service to allow more and more patients to be cared for at home.

The post is 37.5 hours covering 7 days a week between 8am and 6pm, to meet the service requirements. The successful candidate will deputise for the Home First Co-Ordinator, including undertaking incident investigations and presenting findings at the weekly governance meetings.

Main duties of the job

They will also be responsible for the ongoing screening of patients and the identification of care availability to facilitate safe and timely discharge from hospital and provide ongoing support to any ongoing or complex needs within the community through the Discharge to Assess model of care. The successful post holder will work within a supportive multi-disciplinary team to promote the Home First ethos through ongoing therapeutic interventions in the patient’s own environment.

The successful candidate will act as a link between the acute and community services and will work alongside our established Community Resource Team’s in both Wrexham and Flintshire. Qualifications

Essential criteria

  • NMC Registered first level nurse
  • Evidence of ongoing education and professional development to Diploma Level or equivalent level of experience
  • Mentorship & Assessment training

Desirable criteria

  • Management qualification e.g. ILM, RCN Leadership
  • Independent Prescribing
  • District Nurse Specialist qualification or equivalent community nursing / development qualification

Experience

Essential criteria

  • Significant community experience as a Band 5
  • Experience of working with patients who have acute / complex needs
  • Varied clinical experience and multi disciplinary working
  • Significant post registration experience

Desirable criteria

  • Link Nurse role

Essential criteria

  • Analytical skills and ability to interpret clinical signs
  • Organisational skills
  • Demonstrate ability to care and communicate with warmth and understanding
  • Ability to assess, plan, implement, evaluate and delegate appropriately the delivery of care
  • Supervise, teach and assess registered and non-registered staff
