About this job
To be responsible for the delivery of the service.
To manage commissioned service specifications as well as negotiate agreements where there are no (or very old) service level agreements.
To maintain a culture where colleagues feel empowered to support patients, carers and families to live well with their long-term conditions to prevent avoidable health deterioration, hospital admission or unnecessary length of stay through a case management approach.
To act as a key relationship broker with system partners to ensure understanding and appropriate use of the services.
To hold overall accountability for service budgets, with support from Operational Finance Manager and oversight from Deputy Service Director.
To regularly report, to senior managers and commissioners, on the performance of the service, linking in expert advice on professional practice and operational issues within the team.
To work collaboratively with commissioners and other providers to identify needs, evaluate priorities, negotiate and implement appropriate changes in service delivery.
To ensure seamless episodes of care to patients, ensuring they are seen in the most appropriate place in a timely manner acting on feedback as appropriate.
To support the Countywide Service Director and Deputy Service Director on relevant projects/developments including policies, performance, service developments and multi-disciplinary team working.
To lead on the development of, and manage the implementation of, service improvements, in conjunction with Heads of Profession, led by national and local service priorities.
