Job Description:

To deliver a high standard of evidence based inpatient nursing care in the designated caregroup in partnership with the team, the service users and where appropriate their familyTo act as a primary nurse in the assessment of needs and organising of care.To co-ordinate shifts and provide supervision for support workers and students.To work within a team of nurses and other professionals who are committed to the deliveryof a quality service.

To adopt a flexible approach to hours of duty, in order to meet the needs of the service over a 24-hour period.To take charge of a designated clinical area and deputise as appropriate.To represent the Ward Manager and/or care team, at appropriate forums and disseminate information as required.To represent the clinical team perspective at multidisciplinary meetings and clinical reviews of patient care.To be an active participant in the multi-disciplinary team, formulating care plans andutilising appropriate assessments.

Based on Rosemary ward but appointed to the unit and maybe moved to other wards within the unit, To ensure that a comprehensive mental and physical health assessments are completed on admissionTo ensure that all patients have a physical health and mental health care plan and MEWS monitoring in place ensuring regular review of both.Monitor, evaluate and modify physical healthcare interventions/treatments in relation to acute and chronic physical health conditions and to measure progress and ensure effectiveness of intervention(s).To provide a highly specialised range of physical healthcare interventions and knowledge of evidence based models of practice.To take appropriate action when working with vulnerable adults and children following safeguarding vulnerable adult and children procedures, liaising with appropriate agencies.The ability to act promptly and safely by prioritising patients physical healthcare needs and co-ordinating related care.Ensure participation in clinical supervision.To have a basic knowledge of the MHA (1983/2007)To have knowledge of the Mental capacity Act (2005)To promote respect for diversity and combat stigma and exclusion.To communicate with and support relatives and carers and actively promote their involvement with the patients physical care.To be aware of the physical safety and security of the building reporting any problemsTo administer medication within NMC guidelines and Trust’s Medicines Code.
