Role Overview

To work as part of the Bridport Community Mental Health Team. To be responsible and professionally accountable for a patient caseload within the community. The postholder will be working with people with a serious mental illness, providing high quality nursing care and treatment alongside the multi‑disciplinary team.

Bridport Community Mental Health Team is a well‑established multidisciplinary team made up of highly skilled mental health professionals. The team has a compassionate and proactive approach to patient care, strong links with the local community, and is at the forefront of joint working initiatives with primary care and the voluntary sector.

Responsibilities

  • Manage a patient caseload in the community, undertaking bio‑psychosocial assessments.
  • Cover the duty rota and provide evidence‑based interventions with patients.
  • If a registered nurse, rota to the Depot clinic, support the nursing team with blood draws, and manage the Clozaril Clinic.

Requirements

  • Employment requires an Enhanced Disclosure and Barring Service (DBS) check, to be covered by the Trust.
  • Full and valid UK driving licence and use of own vehicle on Trust business.
  • Applicants may present a valid DBS certificate through a subscription to the DBS update service.
