Job Overview
The GMMH rehabilitation service has an exciting opportunity for a dynamic and innovative mental health practitioner to be part of the Home Engagement and Rehabilitation Team. The service is establishing a strong reputation for providing bespoke, intensive and individualised care to service users living with enduring mental health problems to enable them to live their lives to their full potential.
Main Duties of the Job
The post holder will play a central role within the MDT to ensure high quality, recovery‑focused care is delivered to our service users in the community. They will coordinate a small caseload of service users, leading assessment, collaborative care planning and provision of interventions to encourage self‑management, optimise functioning and promote choice and inclusion as part of care.
The service covers a large geographical area including Manchester and Salford (with plans to expand into Bolton this year), so travel across the Trust footprint is expected. The post holder will need a good understanding of the principles of mental health rehabilitation and community mental health care. Extended hours are offered from 8 am‑8 pm Monday‑Sunday, and the post holder will need to be prepared to work on a rota basis across these times. Full induction and training will be provided as part of the role.
The post holder will also be responsible for organising and supervising recovery workers within the team, responding to crises as necessary and delegating tasks to colleagues. They will play a key role in the development of the service, assisting the leadership team in day‑to‑day operations and supervising and supporting junior colleagues.
