This Band 6 role offers an exciting opportunity to provide, advice, and support to patients, families and carers who access our Trust for Orthopaedic care.

You will be working as part of the Trusts Safeguarding and Vulnerabilities Team and will be supported by an experienced Mental Health and Dementia Lead Practitioner who is leading the mental health and dementia service at our Trust.

A current professional registration with NMC, HCPC, or Social Work England is required.

The post involves liaison with other services and external agencies to ensure effective pathways of care.

Sound evidence-based knowledge of the management of people with dementia and mental health needs.

You will be able to work autonomously, exercise clear and coherent decision-making skills and be able to meet the needs of the wider service.

You will be offered regular supervision and support from the Mental Health and Dementia Lead Practitioner.

As well as the ability to develop professionally and gain experience in Orthopaedic care planning.

  • To assist with care planning for patients who are diagnosed with a mental health condition or undergoing diagnosis, which would require a support with care planning.
  • To assist with care planning for patients who are diagnosed with Dementia and or undoing diagnosis.
  • To provide support to the patient’s carers, ensuring reasonable adjustments are in place to support the patient’s needs.
  • Be an advocate for all patients within the Trust under the mental health and Dementia Service.
  • To work in collaboration with the MDT to develop plans of care for both outpatient services and elective admissions ensuring the provision of safe, and effective care.
  • To provide visible frontline support to patients and staff.
