The service is establishing a strong reputation for providing bespoke, intensive and individualised care to service users living with enduring mental health problems to enable them to live their lives to their full potential.

The successful candidate will play a central role within the MDT to ensure high quality, recovery focussed care is delivered to our service users in the community.

The role involves co-ordination of a small caseload of service users, for whom the post holder will lead on the assessment, collaborative care planning and provision of interventions, to encourage self management, optimise functioning and promote choice and inclusion as part of care.

The service covers a large geographical area including all of Manchester and Salford (and with a plan to expand into Bolton this year), therefore there is an expectation that the postholder will travel across the Trust footprint.

You will require a good understanding of the principles of mental health rehabilitation and an awareness of the delivery of community mental health care.

The team has extended hours, offering our service users enhanced support from 8am – 8pm Monday – Sunday and the post holder will need to be prepared to work on a rota basis across these times.

Full induction and training will be given as part of the role.

Main duties of the job

The post holder will be responsible for the co-ordination/ case management of a small caseload of service users. This will involve working creatively within a multi disciplinary team to support service users to identify their own goals and co-ordinate timely and appropriate interventions to help them to achieve these. This includes respecting individuality, values, culture and religious diversity of service users and colleagues and thus contributing to the provision of a service sensitive to these needs.

The post holder will maintain a recovery focus and will demonstrate skill and sensitivity in the assessment of vulnerable service users with higher level of needs.

The post holder will also be responsible for the organisation and supervision of recovery workers within the team. They will be confident in responding to crises where necessary, delegating tasks to colleagues as appropriate. The post holder will play a key role in the development of the service, by assisting the leadership team in day-to-day operations, organising, leading, teaching, supervising and supporting junior colleagues.

Essential criteria

  • Relevant professional qualification
  • Appropriate professional registration

Desirable criteria

  • Relevant post registration training / qualification

Experience

Essential criteria

  • Experience of working in a rehab setting OR a community mental health setting
  • Experience of working with people with complex needs and co-morbidities
  • Experience of conducting assessments identifying health and social care needs.

Desirable criteria

  • Experience of working across both inpatient AND community settings
  • Experience of delivering line management and clinical supervision
  • Evidence of acting as educator for students
  • Experience of funding application/social care/CCG
  • Experience of leading safeguarding enquires

Knowledge

Essential Criteria

  • Knowledge and understanding of recovery model, appropriate therapeutic approaches and engagement strategies
  • Knowledge and understanding of Mental Health Act, Mental Capacity Act & Care Act
  • Working knowledge and understanding of CPA

Desirable criteria

  • Understanding current national policy, initiatives, and principles of mental health rehabilitation
  • Awareness of current developments within GMMH rehabilitation division

Skills

Essential criteria

  • Be able to manage a caseload and work autonomously within the team
  • Evidence of good time management and organisational skills
  • Ability to assess, plan and evaluate care
