Discharge Liaison Mental Health Practitioner
Closing date: 13 April 2026
Exciting opportunity to play a pivotal role in enabling safe and timely discharge, through active discharge planning from the point of admission to reduce delayed transfers of care.
Effective discharge planning provides a multidisciplinary approach to continuity of care; a process that includes identification, assessment, goal setting, planning, implementation, coordination, and evaluation. Effective discharge planning supports the continuity of care, providing a critical link between treatment received in hospital by the patient, and post‑discharge care provided in the community.

- Preparing agreed assessment reports/treatment plans accurately in the accepted format.
- Making applications for funding discharge packages based on MDT recommendations.
- Maintaining adequate records as required by existing procedures, entering appropriate details on service users’ case notes as necessary, and showing willingness to attend training to facilitate recording information electronically.
