This is a unique opportunity to make a real difference by helping her settle safely and confidently into her home environment.She lives with complex long-term conditions including chronic pancreatitis, hypermobility/Ehlers-Danlos features, chronic pain and neurodivergent needs. Nurses will play a key role in delivering structured, person-centred care and supporting her independence.Youll provide holistic nursing care including:Complex medication management (subcutaneous and jejunal routes)Bowel care and pain managementRespiratory support and safe mobilityCalm, consistent communication and routine-based careSupporting daily living tasks such as light household duties and feeding her catsYou will have HCA support during the day (08:0020:00) to assist with mobility, supervision, and household tasks, ensuring you can focus on clinical care.Youll be supported by clear protocols and a Nurse Practitioner-led team to ensure you feel confident and equipped in your role.Why This Role Matters This is more than a nursing jobits about building trust and stability for someone who values clear communication and consistency.
