Job Description: Part Time Performer Dentist required 3 days per week in a busy, well established, independently owned practice near Thornton, West Yorkshire.

• Weds, Thurs & Fri
• 3900 UDAs available
• £14-£14.50 per UDA
• 50% split on Private & Labs
• Circa £100,000 Take Home for 5 days
• Stable & Established patient list inherit
• 2 Fully computerised surgeries, R4
• Digital x-rays, Wave One Endo, Apex Locators
• Hours 9am-5pm
• Full support team including a Hygienist
• 15 minutes to Bradford, 45 mins to Leeds
• Available now, start date negotiable

• Full NHS Performer Number if applicable
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate

• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
