Job DescriptionAs a Community Care Professional, you will provide essential care and support to clients in the community —enabling clients to maintain dignity, independence, and quality of life within their own homes;Key ResponsibilitiesDeliver high-quality, person-centred care in clients’ homesSupport with daily living tasks such as washing, dressing, mobility, light household duties and meal preparationProvide companionship and build positive, trusting relationshipsAssist with medication prompts and general wellbeing monitoringAccompanying your client to appointments, or a visit to the shopsRecord and report care activities accuratelyPromote independence, respect, and choice in every interactionCommunicate effectively with clients, families, and team members to ensure seamless care coordinationSupport clients in maintaining their independence and social engagement within the communityQualificationsAdditional InformationWhat we provide:One hour minimum visits with clients to allow time to care, complete tasks without rushing and get to know your clients.Matching of Care Professionals and clients based on mutual interests and hobbies, so there is never something short to talk about and you can provide more personalised support.
