Mental Health Nurse Practitioner, Hope Service (B6-7 Dev. Role)

Overview

This innovative and exciting multi‑disciplinary service is provided by Health, Social Care and Education professionals to young people aged 11‑18 with mental health, social, emotional and behavioural difficulties, living in Surrey. We are a Centre of Clinical Excellence and have been awarded Outstanding by Ofsted.

Role

We are seeking a Band 6 to 7 Mental Health Nurse Practitioner to work at the Hope service. The role involves assessing and supporting young people with complex mental health, behavioural and social care needs.

Responsibilities

• Provide mental health assessments focusing on risk management and the needs of the young person.
• Deliver appropriate interventions and/or sign post to services for the young person.
• Offer consultation to the multidisciplinary/multi‑agency Hope Service team on mental‑health related issues.
• Participate in crisis/urgent assessments, care planning, risk assessment and the development of risk management and treatment plans.
• Facilitate diversion from T4 admission or out‑of‑area placement, supporting recovery within the young person’s home or community.
